Superbill for Cash Pay Physical Therapy: What to Include

A superbill for cash pay physical therapy is an itemized receipt that carries your identifiers, the patient’s details, the date and nature of the service, a diagnosis, the fee and proof of payment, so the patient can ask their own plan for out-of-network reimbursement. You do not bill the plan, and you promise no payback. Treat it as a documentation habit that goes with a cash practice in a private room and costs nothing to produce.
The field list
| Field | What goes there | Why a plan wants it |
|---|---|---|
| Provider block | Your name, credentials, license number, practice name and address | Identifies who delivered care |
| National Provider Identifier | Your individual NPI, and a tax ID if you use one | Standard claim identifiers |
| Patient block | Name, date of birth and, if you collect it, the plan’s member ID | Matches the claim to the member |
| Date of service | One line per visit | Each visit is a separate item |
| Service codes | The procedure codes for what you did, such as evaluation or therapeutic exercise | Tells the plan what service was provided |
| Diagnosis codes | The treatment diagnosis you recorded | Connects the service to a reason for care |
| Place of service | A code describing where care took place | A rented room is a non-facility setting; confirm the right code with your billing resource |
| Time and units | Minutes or units, as your coding requires | Some codes are time-based |
| Fee and payment | The fee charged, the amount paid and the method | Proof the patient paid |
| Signature and date | Yours | Attests to the content |
Coding choices are a professional judgment, so use the codes your own documentation supports and ask a billing resource when unsure.
What the superbill must not do
- Never list a service you did not provide, or one your note does not support. The superbill should match the chart, line for line.
- Never promise a refund. Put a sentence at the bottom: “This document records services and payment; reimbursement is determined by your plan.”
- Never list a diagnosis you did not document. The treatment diagnosis on the superbill belongs in the evaluation.
Build the habit into the visit
- At intake, tell the patient how a superbill works and that you do not submit it.
- After each visit, record the date, service, time and fee in your documentation system.
- Issue the superbill on a schedule, either per visit or monthly, as a PDF from your own software.
- Keep a copy with the chart. The HIPAA in a gym-based practice answer covers what stays on the provider when you share records.
If a patient started care under direct access, note the date care began; the rules for direct access and referrals are laid out in the Indiana 42-day direct access answer, and some plans ask for a physician referral to reimburse regardless of state law.
Pricing and receipts together
A superbill is easier to write when your fee structure is simple. Set a price per visit, per length and per type, then use the same labels on the receipt. The cash PT pricing guide walks through how a clinician builds those numbers. FlexWerk rents private rooms at Carmel City Center by the hour, from $18 to $22, with no lease or membership, and the first hour is free for fitness professionals through get started. Each PT sets their own fees, and the facility provides space and no clinical or billing services.
One-page template to build today
Write your template once: header block, patient block, a table of dates and services, a totals line and the reimbursement disclaimer. Run one test superbill for a friend as if they were a patient, and check it against your chart. Then stop adjusting and use it.
Related questions
Does a superbill mean the patient gets reimbursed?
No. A superbill is a record of what happened and what was paid. Whether any money comes back depends entirely on the patient's own plan, which is why you should say so in writing at the first visit.
Should the superbill go to the insurer from my office?
In a cash practice the patient submits it, not you. Once you submit claims you are billing the plan, which is a different practice model with its own contracts and rules.
When should I hand it over?
Many therapists issue one after each visit or at the end of each month, and keep an identical copy in the record. Choose one rhythm and state it in your intake paperwork.
