Do Athletic Trainers Need a Physician Referral in Indiana?

Physical therapist guiding a young woman through a knee rehab exercise, the kind of care referral rules for athletic trainers cover

In Indiana, athletic training is a licensed practice under IC 25-5.1, and the law ties that licensed work to the direction of a physician, so an athletic trainer should not assume private clients can be seen the way a coach sees them. The practical answer for most private settings is that the licensed services need a physician relationship, while strength and conditioning coaching does not. Before you advertise, confirm the exact wording with the state and plan the paperwork, and the room where you will see clients, around it.

What the rule says

Indiana regulates athletic trainers through Indiana Code Title 25, Article 5.1, with licensing rules in Title 898 of the Indiana Administrative Code. The Indiana Professional Licensing Agency lists both on its athletic trainers page, which I checked in October 2026. Those sources describe athletic training as practiced under physician direction, and the rules also expect the trainer to consult the referring practitioner when treatment seems contraindicated and to inform them when an athlete’s status changes. The full statute text was not retrievable from an official page when this article was written, so the rule is described in general terms with no exact thresholds; read the current text on the agency site before relying on any detail. This is general information, not legal advice, and rules change.

Two services, two rulebooks

Most private athletic trainers sell two kinds of work, and each answers to a different rulebook.

Service Typical examples Rule that applies
Licensed athletic training Injury assessment, rehabilitation and reconditioning of athletic injuries The athletic trainer statute and rules
Coaching and fitness Strength programs, conditioning, general movement prep Indiana does not license personal trainers

The trouble starts when a flyer or intake form blurs the two. If a family believes they are receiving athletic training when they are actually buying fitness coaching, or the reverse, the trainer is exposed. Label each service clearly, and keep separate agreements and records for them.

What it means outside a school

A school setting usually has a physician relationship built in, through team doctors or district policies. A private suite does not. Taking private clients means you must establish that relationship yourself:

  1. Identify a supervising or referring physician, or a clinic that sends you athletes.
  2. Put the working arrangement in writing, including who is responsible for what.
  3. Document referrals, communication and any change in an athlete’s status.
  4. Decline licensed services you are not able to deliver within the rules, and refer out.

The private clients answer walks through the same two-rulebook idea with a practical checklist, and the school contracting page shows how school work differs from independent work.

Questions to ask before you take a private client

Use this list as a gate:

  • Who is the physician involved, and have they agreed to this role?
  • Is what the client wants licensed athletic training or fitness coaching?
  • What do I document, and where do I keep it?
  • What is my emergency plan if something goes wrong in the room?
  • Who do I call if the situation is outside my scope?

If you cannot answer a question, pause. A phone call to the Professional Licensing Agency or an attorney who knows health care licensing costs less than a complaint.

Common mistakes

Trainers sometimes assume an expired school relationship still covers them, advertise services without naming scope, or let a parent’s request override a physician’s guidance. Another mistake is to describe a coaching service with clinical words. Use plain terms for what you really deliver, and keep clinical language for work within your license.

Where a private room fits

A rented room is a place, not a legal status. FlexWerk spaces rent by the hour at Carmel City Center, with no lease, and the first hour is free for fitness professionals; each professional remains responsible for their own license, insurance and scope. Plan the physician side first, then book the room for the evenings you will see clients. For emergencies in a session, see what to do if a client gets injured.

Related questions

Does a physician referral apply to coaching and fitness work too?

Indiana does not license personal trainers, so general strength and conditioning coaching is a different service from athletic training. The licensed work is where the athletic trainer rules apply.

Where do I confirm the current rule?

The Indiana Professional Licensing Agency's athletic trainers page lists the governing statute and rules, and the agency can answer practice questions. Check there before you launch a private service.

Does renting a private room change the rule?

No. The room is only a location. Your license, the services you perform and any physician direction that applies stay the same wherever you work.
